Work Permits

Application for Minor Work Permit
This application should be completed by the student, the student's legal guardian and the student's prospective employer. The completed application should be submitted to the school in which the student is enrolled. If the student is enrolled in an alternative education setting (home schooled, GED) this application should be returned to the school district in which the student resides.

Physician's Certificate for Minor Work Permit
This form should be completed by a physician before a minor applies for a minor work permit.
If the student has previously submitted a physical for an organized school activity within the school year, this form is not necessary.

Minor Wage Agreement
Ohio Revised Code, Seciton 4109.10 requires a written agreement to wages between a minor and their employer.
The following form is furnished as an example, and may be reproduced by any employer. This form should be prepared in duplicate and signed by both the employer and the minor. One copy to be given to the minor and the other copy to be retained by the employer in the personnel file of the minor.

Parent or Guardian Consent form
Minors aged sixteen or seventeen who are to be employed during summer vacation months after the last day of the school term in the spring and before the first day of the school term in the fall, in nonagricultural and nonhazardous employment as defined by the ''Fair Labor Standards Act of 1938'', 52 Stat. 1060.29 U.S.C.A. 201, and similar state statutes, or in other employment not prohibited to minors age sixteen or seventeen by law, shall not be required to
provide an age and schooling certificate as a condition of employment. In order to be hired for employment during summer vacation months, such minors shall provide the employer with the following:- Evidence of proof of age in the same manner as proof of age is provided the superintendent of schools under division C of section 3331.02 of the Revised Code.
- A statement signed by the minor's parent or guardian consenting to the proposed employment during the summer vacation months. For the purpose of this section, in the absence of a parent or guardian a person over eighteen years of age with whom the minor resides may sign such statement.
The employer shall retain a copy of the proof of age and the statement of consent with the minors employment records.

Our Welcome Center gives families a central location to answer questions, provide district information, process online registrations, and help you with enrolling your child in Pickerington Schools.

Change of Student Information Form
Any changes to a student’s home address or household address must be requested on the district’s Change of Student Information form and processed through the Welcome Center.

Shared Residency Affidavit
This form is valid for the 2024-2025 school year only. A new form must be submitted every year. Renewals must be received in this office no later than August 1, 2024. Use this form to certify that you have established residency within the Pickerington Schools district and are not maintaining a separate residence elsewhere. Submit this form and its supporting documentation to Welcome Center, 130 Hill Rd S, Suite 102, Pickerington, OH 43147.
